Once you reach step 42 on Volcano Island of the main quest, the old woman becomes sick. She tells you that she could really do with a little help around the house. This inspires Baibai to create the Little Robot.

You can create a Little Robot for 2000GP, equip it and talk to the old woman to give it to her.
